Whether a minimally invasive or an open approach is used depends on the pathology and the individual patient rather than on preference. Intraoperative neuromonitoring is used to protect nerve function throughout. It is worth saying plainly that most back and neck pain does not require surgery. Conservative treatment and physiotherapy resolve the majority of cases, and an operation becomes appropriate only when symptoms persist despite them, or where there is evidence of nerve compression that will not improve on its own.
